Medical Debt Is Different. Here's How to Fight It.

Roughly 100 million Americans are currently carrying some form of medical or dental debt. Depending on how you measure it, that's somewhere between a third and 40% of working-age adults — and the total owed is estimated at more than $220 billion.

Medical debt looks like other debt on the surface: a bill you owe, that shows up in collections if it goes unpaid. But it behaves differently, gets created differently, and — unlike a credit card balance you chose to run up — it's usually the result of something that happened to you, not a spending decision you made.

Why medical debt is different

  • You rarely agree to a price in advance. With almost every other kind of debt, you know the cost before you take it on. With medical care — especially emergency care — you often can't.
  • The bill can be wrong. Medical billing errors are common: duplicate charges, incorrect codes, services never received, or insurance that should have covered something but didn't get billed correctly. A meaningful share of medical debt is at least partly disputable if someone takes the time to check.
  • It can hit anyone. Having insurance reduces medical debt risk but doesn't eliminate it — high deductibles, out-of-network surprise bills, and gaps in coverage mean insured people carry medical debt too.

The credit reporting rules have changed — recently and repeatedly

This is worth knowing specifically: starting in 2022 and 2023, the three major credit bureaus (Equifax, Experian, and TransUnion) made voluntary changes to how they handle medical debt. Paid medical collections no longer appear on credit reports at all. Unpaid medical collections don't show up until a full year has passed — giving people time to negotiate, dispute, or arrange insurance coverage before it can touch their score. And the reporting threshold for medical collections was raised, so very small balances generally don't get reported.

Federal regulators have separately pushed to go further and remove medical debt from credit reports altogether, though the exact rules in this area have continued to shift and face legal challenges — so if you're dealing with a specific medical collection, it's worth checking current guidance from the Consumer Financial Protection Bureau rather than assuming an older rule still applies exactly as written.

How to actually fight a medical bill

  • Request an itemized bill before you pay anything. Hospitals are required to provide one, and itemized bills surface errors that a summary bill hides.
  • Ask about the hospital's financial assistance ("charity care") policy. Most nonprofit hospitals are legally required to have one, and many patients who'd qualify never ask.
  • Negotiate the price, not just the payment plan. Medical bills are notoriously negotiable, especially if you can offer a lump-sum payment at a discount, since providers often write off the balance anyway if it goes to collections.
  • Appeal denied insurance claims. A large share of appealed claims get at least partially overturned — most people never appeal in the first place.
  • Don't put it on a credit card by default. Moving medical debt onto a card converts a 0%-interest hospital bill into a high-interest revolving balance. Explore a hospital payment plan or hardship program first.

The bottom line

Medical debt isn't a personal failing — it's a predictable outcome of a system where prices are opaque and errors are common. It also comes with more consumer protections than most people realize, and more room to negotiate than almost any other kind of bill. Before you pay a medical bill in full or put it on a card, it's worth spending twenty minutes checking whether you actually owe all of it.
